It was Orville Wright, who survived the crash. The passenger and casualty was Lt. Selfridge of the US Army.
A couple of years and a few casualties later in Britain the Honorable C.S. Rolls, co founder of Rolls Royce crashed and died. Flying was dangerous back then!
